How do employees enroll in the Plan?
Employees can enroll in the Plan in one of the following ways:
•Meet with a local representative in-person to review financial goals and/or get help in completing the enrollment paperwork.
•Enroll by Internet. Employees can access the Plan web site and select the enrollment tab. Employees can complete the enrollment process online, including selecting their investment choices.
•Complete the EZ Enrollment Participation Agreement Form and submit to Human Resources.
Employees will need to include contact and personal information, deferral amount and designated beneficiary(ies). The investment vehicle will be automatically set to one of the lifecycle portfolios based on their date of birth. Investments can be changed at any time once an employee is enrolled.
