Prevailing Wage - Labor Compliance

The following forms are all required and must be submitted at the start of each contract with the City. Labor Compliance forms are only submitted one time to the Office of Equality Assurance (OEA).

1. Fringe Benefit Statement

This form specifies the benefits you provide to workers. You are required to submit verification documents to receive credit for any benefits you provide.

2. Workforce Statement

This form specifies the workers you plan to use on a project. The work classification assigned to a worker must match the classifications on the DIR website.

3. Certification of Posting and Distribution (Prime Only)

California Labor Code requires the current copy of the General Prevailing Wage Determination must be posted at a public works construction job site. Use this form to certify the current General Prevailing Wage Determination has been distributed and posted.

4. List of Subcontractors

This form will allow you to identify each subcontractor and their duties.
